Part of the problem stems from a lack of clarity in what is meant by
schizophrenia. In a study of 479 first episode psychotics we found 86% met
criterion A for DSM schizophrenia (i.e. the troubling symptoms for
themselves & others) but only 32% were classified as schizophrenic in DSM.
We also found DSM-III-R predicted by duration of illness & affective
exclusion much better than the prediction from psychotic symptoms. Not
surprisingly we couldn't get it published in the US. [Bell et al, (1998)
