HTML 2.0 reconstruction done

I am now finished with what I planned to do (plus all the other fixes
I found necessary in the process). The result is now available from both

Someone needs to test the DTD.

I'm sorry that I could not satisfy everyone's desires regarding the
issue of user agents accepting bad HTML, but it is a bit late now to
be changing a fundamental aspect of the text/html media type.
We can put that on the list of issues for 2.1, but in my opinion
it is clearly inappropriate to do so for 2.0.

Of course, you are welcome to try and convince Dan otherwise.
Speaking of whom.... Dan, are you ready to take the author's chair again?
I'll finish my notes on the MIF -> text conversion process when I wake up.
