For a while, I toyed with the idea that anchor names
shoud be SGML IDs. There are several conflicts:

	* IDs have to start with a letter
	* IDs have to stick to name characters (letters, digits, period, hyphen)
	* fixed length limit
	* IDs aren't case sensitive

In the future, when we start allowing ID's on lots of elements,
and in general, making better use of the SGML ID feature, that
will be the time to worry about such things. For now, I'm taking
out all the crud that talked about anchor names as SGML IDs.
