It doesn't really matter whether the insertions and deletions are internal or external when the point is to display the document - at thtat point, presumably, your external tool compares the requested versions in the database and applies the text styling in the version to be displayed. Gotta have a way to indicate the styling once the tools has figured out what to mark how...
